Our Outcomes

We are outcome based service and work with all individuals and agencies involved to agree for long term success goals.
We offer special treatment and management programs in non secure accommodations and least restrictive environment to bring significant long term care and cost benefit for referrers. Our strategies aim at preventing long term hospital stays and re admissions.

We help our service users

  • Minimise symptoms while improved mental health problems to gain optimum functions and quality of life
  • Aim at Identifying the level of care required in current and future placements, including legal frameworks of care if needed
  • Define and Develop placement profile, with manageable recommendations about location, environment, staffing needs, clinical needs i.e., psychiatry, psychology, Speech & Language Therapy, Occupational Therapy, care and nursing
  • Working together wit referrers and families, plan for Discharge planning, including relapse prevention strategies, crisis contingency and early warning of relapse and intervention and recovery plans
  • Our domains of Outcome Measures are based on all health and social domains

E.g.:-

  • Management and reduction of positive symptoms such as hallucinations and delusions
  • Reducing the impact of negative symptoms such as apathy and withdrawal
  • Rationalising and minimising use of psychotropic medication – through use of NICE guidelines and interventions
  • Reducing inequalities in health care through liaison with primary care services
  • Reducing the need for services through improved self agency, moving patients to less intensive services or away from secondary care
  • Ensuring adequate financial position of users by supporting them with claiming their benefits and budgeting skills
  • Supporting vocational and educational activities
  • Improving activities of daily living and social functioning
  • Supporting and educating carers and families, and ensuring the welfare of children deemed at risk
  • Supporting service users to make informed decisions and to ensure provision is in place to facilitate this neutrally where a service user does not have capacity (use of Mental Health Advocacy)
  • Developing the stakeholder role of service users and forming effective relationships with partner agencies
  • Ensuring appropriate support with seeking of accommodation, supporting accommodation and providers
